John Steel's Truth Lies and Advertising is still the first book anyone should read on planning. Much might have changed since it was written, but to be honest, the majority of stuff is the same.
Yuu really should take a look at this talk he did recently and pay close attention to the ditinction between doing something becasue you can, instead of doing what you should.

First of all I want to say terrific blog! I had a quick question that I’d like to ask if you don’t mind. I was interested to find out how you center yourself and clear your thoughts prior to writing. I’ve had a hard time clearing my mind in getting my ideas out. I truly do enjoy writing however it just seems like the first 10 to 15 minutes are generally wasted just trying to figure out how to begin. Any ideas or tips? Thank you!
